Overview

The Clinical Pharmacist, Long Term Care will support a team of Long Term Care Consultants/Pharmacists to meet the contractual requirements with each facility, based on corporate agreements.  Responsibilities include coaching and developing the Consultants to provide superior customer service and health care needs. 

Job Description

  • Provide input to Pharmacy Store Manager on individual consultative activities for each Pharmacist in relation to feedback from the long-term care sites
  • Provide monthly and quarterly reports on Consultant activities to Pharmacy Store Manager and facility administration
  • Provide clinical scheduling recommendations to Consultants to meet contractual requirements based on fiscal objectives
  • Provide regular updates to Pharmacy Store Manager on issues, resolutions and successes for each site
  • Focus on clinical services (i.e. disease state management, medication reviews, health risk assessments)
  • Ensure prescription safety prioritizing technical functions to Pharmacy Assistants (as required)
  • Initiate care plans, follow up, and proper documentation
  • Collect relevant patient, drug and disease information
  • Identification of actual and potential drug related problems through completion of medication reviews
  • Monitor medication adherence
  • Provide patient counseling for new therapy and provide education to patients with an individual medication review
  • Tailor counseling to the needs and concerns of the patient (i.e. literacy, hearing loss, privacy, etc.)
  • Acknowledge customers promptly and professionally
  • Demonstrate warmth, empathy, and compassion towards the customer
  • Provide superior customer service to every customer
  • Responsible for understanding all service offerings and providing each customer with the program that meets their health needs (i.e. Pill Pack Plus, etc.)
  • Other duties as required

Job Requirements

  • Bachelor of Science in Pharmacy from a recognized institution
  • Must be a Licensed Pharmacist in the province of responsibility
  • Experience in Long Term Care Consulting
  • Injection Certification
  • Above average communication skills (oral and written)
  • Strong organizational skills and attention to detail
